Summary : Introduction: This study aimed at evaluating the perceptual and objective aerodynamical results of the surgical technique of the posterior pharyngeal wall augmentation with autologous fat graft in the treatment of velo-pharyngeal insufficiency in children. Materials and Methods: This is a retrospective study of patients undergoing this technique. The results of the phoniatric examination were collected preoperatively and at least 3 months after surgery. Results: An improvement in nasality was present for both perceptual and objective analyses for 9/10 patients. One patient did not present lasting improvement in his voice and underwent a classical technique of pharyngoplasty with flaps. Conclusion: The technique of the autologous fat graft augmentation of the posterior pharyngeal wall is minimally invasive, and reliable in the context of mild to moderate velo-pharyngeal insufficiency. The importance of speech therapy pre- and post-operative is emphasized, as well as the collaboration with a phoniatrician who performs the perceptual and objective analyses of the voice, and the anatomical assessment (nasofibroscopy) used to determine the surgical indication.
