Summary :
The transmeatic fully endoscopic surgery of the cholesteatoma of the middle ear needs a precise assessment of the extension of the disease in the antromastoid. This pre-operative assessment is at best performed with the CT-scan of the temporal bones. This case-report is original because of the annular image in the antrum, which did not allow to augur some extension of the cholesteatoma, identified during the surgical procedure.
