The eye movement autophony: An intriguing symp­tom in superior semicircular canal dehis­cence syndrome

Summary : Introduction: In the superior semicircular canal dehiscen­ce syndrome (SSCDS) several auditory and vestibular signs and symptoms could be found, including the eye movement auto­phony. Herein, we report the case of a patient with a SSCDS and discuss primarily the eye movement autophony symptom. Case report: Female, 33 years old, has been complaining of hearing the eye movement for 5 years. Also reports aural fullness in the left ear. The patient had no dizziness and the clinical examination was normal. Audiometry tests were performed with impedance tests, vestibular evoked myogenic potentials and temporal bone CT scan, which confirmed the diagnosis of superior semicircular canal dehiscence. Discussion: The rare manifestation of eye movement autophony is a typical symptom of SSCDS, not found in other 3rd window diseases. Other signs and symptoms of the SSCDS can mimic various entities of the middle and inner ear. The eye movement autophony may be explained by the transmission of vibration, from the extrinsic muscles of the eye, through the venous sinuses to the dehiscence in the superior semicircular canal. Conclusion: Despite rare and unconventional, the eye move­ment autophony is a potential symptom in SSCDS and can be plausibly explained.
